Detailed Overview

Under the direction of a senior position such as a Physiotherapist, Occupational Therapist, Registered Nurse or Recreation Therapist, assists designated clients who have severe and persistent mental illness with assigned rehabilitative activities including activities of daily living; implements rehabilitative treatment plans and programs and assists the therapist in monitoring and evaluating clients' progression; transports and accompanies clients on appointments and activities.
Responsibilities

  1. Carries out designated components of a rehabilitative treatment plan such as actively involving clients in day-to-day activities, demonstrating and participating in activities of daily living and providing functional skills training to clients and families in performing life skills such as communication, maintaining personal hygiene and appearance, meal planning, food preparation, safety, shopping and budgeting.
  2. Works collaboratively with the interdisciplinary team in the development of individualized rehabilitative treatment plans with clients and families by observing and identifying client needs, skills, interests and goals; gathering appropriate history, and collecting relevant records and documentation.
  3. Contributes to the overall evaluation of the client by methods such as observing client's behaviour and symptoms, documenting observations in accordance with standard procedures or specific directions, providing verbal/written reports on changes in client's appearance and/or health, discussing/recommending changes to treatment plan recognizing and responding to anticipated/unanticipated client responses to care, and reporting significant observations to a nurse or Program Coordinator.
  4. Arranges, plans and accompanies clients on appointments, errands and activities such as shopping, banking, bill payment, medical appointments, viewing accommodations and social/leisure activities by methods such as contacting relevant organization/business/service provider and assisting client with completing/processing forms, in order to further the development of self management skills.
  5. Provides or assists clients with personal care as required such as bathing, grooming, dressing, oral hygiene, toileting needs, perineal care and incontinence supply changes in accordance with established procedures by methods such as referring to treatment plans.
  6. Transports/escorts residents to programs, appointments, services and other activities by operating a passenger vehicle. Books client trips, alters schedules, arranges time for pickup and dropping off of residents.
  7. Participates in team meetings and/or conferences by methods such as providing input, presenting relevant material, acting as an advocate for the goals and objectives of the program, and promoting optimal support for the clients.
  8. Engages in on-going team planning, development, evaluation and quality improvement activities that facilitate team goal setting, problem solving and collaboration by attending meetings and in-services, providing input/recommendations to team members regarding efficiency of processes/procedures relating to work duties.
  9. Performs other related duties as assigned.

Qualifications

Education and Experience

Completion of Rehabilitation Assistant Program or an equivalent combination of education, training and experience.

Valid Class 5 BC Driver's License with access to personal vehicle for business purposes.

Food Safe Level 1.
