Job Profile:

Lions Housing Centres Inc. is currently recruiting for Full-Time Permanent 1 EFT Rehabilitation Aide position for the Personal Care Home facility. As a Rehabilitation Aide, you will be responsible for assisting with the establishment, execution, and monitoring of rehabilitation programming in conjunction with the Occupational Therapist and the Resident Care Managers from designated areas. This position reports directly to the Director of Care.

Job Responsibilities:

Resident Assessment and Follow-up

  • Interact with residents to support functional activities such as positioning, graduated weight bearing, transfers, seating, exercise, and walking, as directed by Occupational Therapists and/or the Care Team.
  • Assist in the rehabilitation of residents experiencing acute changes under the guidance of Physical and/or Occupational Therapists.
  • Aid in developing care plans for residents at risk of pressure ulcers.
  • Maintain an updated list of residents involved in therapeutic interventions, including walking programs.
  • Facilitate assessments to determine transfer/lift status.
  • Implement and maintain a standardized transfer/lift decal system throughout the facility.
  • Develop and manage therapeutic/restorative walking and exercise programs in collaboration with Occupational/Physical Therapists.
  • Ensure safe and appropriate use of seating systems and accessories.
  • Monitor and report changes in physical, mental, and emotional status.
  • Participate in required care conferences.
  • Document interventions, observations, and care according to established policies using manual and computer systems.
  • Assist with transfers, meal assistance, and activities of daily living (ADL) as needed.

Equipment/Inventory Assignment and Maintenance

  • Develop and maintain a detailed inventory of facility, resident, and agency-owned equipment.
  • Perform minor repairs on mobility aids.
  • Coordinate a formalized preventative maintenance program internally and with external agencies.

Quality Improvement/Risk Management

  • Educate on bed and wheelchair mobility principles.
  • Report all potential health and safety hazards as per facility policy.
  • Participate in clinical issue working groups.
  • Monitor care through quality improvement audits.
  • Implement Fire and Disaster Plan, security policies, and resident safety procedures.
  • Adhere to Workplace Safety and Health policies and procedures, including job hazard analysis.
  • Promptly report concerns regarding resident care, unit functioning, or staff performance to Registered Nurse or Resident Care Manager.
  • Perform other duties as assigned.

Your education and qualifications:

  • Complete high school education, Manitoba standards, required.
  • Recognized Rehabilitation Assistant Certificate required.
  • Minimum two years of recent experience as a Rehabilitation Aide or relevant demonstrated experience.
  • Experience in long-term care settings preferred.
  • Strong interpersonal and customer service skills.
  • Good attention to detail.
  • Knowledge of employment standards and health & safety.
  • Basic computer skills required.
  • Ability to take initiative, work autonomously, and prioritize tasks effectively.
  • Must be proficient in both oral and written English communication.
  • Ability to work independently and be flexible to change as situations warrant.
  • Good interpersonal skills and an expressed willingness to work as a member of an interdisciplinary team.
  • Organizational abilities, with a particular emphasis on time management.
  • Satisfactory Criminal Record with Vulnerable Sector Search.
  • Satisfactory Adult Abuse Record Check.
  • Up to date immunization.
