Horizon Health Network (Horizon) is seeking a strategic, innovative, and detail-oriented individual to join our team as a Process Improvement Facilitator, supporting strategic planning. The ideal candidate will have experience in strategic planning, performance measurement, change management, and continuous improvement.

Who you are
You have a worked in healthcare or other industry in a process improvement, planning or leadership role for a minimum of 5 years with progressive work experience. You have experience in project management, planning, change management, and strategy execution and performance measurement. You have a proficiency level in Microsoft Office applications. You have strong analytical, organizational, problem solving, and communication skills.
Education Requirements:
- University degree in Business Administration, Science, Engineering or a related discipline with six years of progressive work experience
- Completion of Lean Six Sigma Green Belt or Black Belt training
- Change Management certification
Written and spoken competence in English is required at an advanced level.
What you will do
Responsibilities include:
- Work with stakeholders to identify key performance indicators for the ongoing measurability and monitoring of implemented performance changes
- Support the annual strategic and operational planning processes
- Provide guidance and strategic direction to ensure alignment of initiatives to Horizon's key priority areas
- Support programs on the development of operational plans that align to the organization's strategic goals
- Guide planning and identification of projects and initiatives to achieve strategic priorities
- Ensure strategic priorities are aligned to performance metrics
